Prevention and Treatment of Pressure Ulcers: Skin Assessment and Preventive Skin Care – an extract from the Clinical Practice Guideline
This guideline extract focuses on prevention of pressure ulcers. The extract includes three chapters: Skin and Tissue Assessment, Preventative Skin Care and Emerging Therapies for Prevention of Pressure Ulcers. The Emerging Therapies chapter covers microclimate, prophylactic dressings and fabrics and textiles. Skin and tissue assessment underpins the selection and evaluation of appropriate preventive interventions. This chapter comprises recommendations on skin assessment, including the identification and differentiation of erythema, components of a comprehensive skin and tissue assessment and considerations when assessing individuals with darkly pigmented skin. The preventive care includes recommendations on protecting erythematous skin, avoidance of massage, promotion of skin hydration and protecting the skin from excessive extrinsic moisture. Other preventive measures, including microclimate control, prophylactic dressings and use of low friction textiles and fabrics are also discussed in this extract.
